GLOBAL RISK CLINIC

Most major
incidents aren't
single failures.
They're a chain
of small ones.

A warning isn't acted on. A control is bypassed. Information doesn't reach the right person. One by one they look manageable.

What small signals are visible in your organisation right now that could become a bigger problem?

Spot.
Challenge.
Act.
Prevent.
